• Your department & contact information
• Date(s), Time(s), and Location(s) of your request
• Case Information – caption and number
• Special Requirements (e.g. Rough ASCII, Realtime, Reporter SSN, Contact Information, or Security Clearance)
• Turn around time – anything up to and including same-day
• Attorney information (if applicable)
• Special Invoicing or Contract Instructions
